Pediatric Residency: Inpatient Clinical Experiences

Postgraduate Year 1 (PGY-1)

Intern year (PGY-1) is dedicated primarily to the inpatient experience at Children’s Hospital at Montefiore Einstein (CHAM), where interns serve as their patients’ primary clinicians, under the supervision of senior residents and attendings. The three inpatient floors are divided into teams by age group or by subspecialty, offering a unique learning environment for interns in which they have the opportunity to manage a variety of both common and complex pediatric disease processes as they occur at different developmental stages.  

In addition to the inpatient experience, the intern year consists of a variety of outpatient experiences, including a month of Behavior and Development, an elective month dedicated to a subspecialty of the intern’s choice, as well as rotations through subspecialty clinics. One afternoon each week, every intern and resident takes care of their own patients in continuity clinic.  

Interns spend one month each in the newborn nursery (NBN) and the Neonatal Intensive Care Unit (NICU) at the Jack D. Weiler Hospital on Einstein Campus. While at Weiler Hospital, in addition to healthy newborn care, interns learn and practice neonatal resuscitation skills, attend deliveries and become familiar with the management of common problems in the NICU.  

In addition to learning clinical management skills, interns begin to develop as teachers of third- and fourth-year medical students.

Postgraduate Year 2 (PGY-2)

Building on the knowledge base established during intern year, the PGY-2 year is devoted to cultivating skills for independent clinical decision-making, team leading, and teaching. PGY-2s are the supervising residents on inpatient units, conducting rounds and guiding interns and medical students through patient management. Inpatient units include General Pediatrics, Hematology/Oncology, Adolescent Medicine, as well as teams that cohort Cardiology, Gastroenterology, surgical subspecialties, Renal, and Bone Marrow/Renal/Liver Transplant patients. PGY-2s also spend time in the Pediatric Critical Care Unit (PCCU), gaining exposure to critically ill patients. A crucial component of PGY-2 is increased elective time, which allows residents to explore their personal interests in the pediatric subspecialties, general pediatrics, private practice, and research.

Postgraduate Year 3 (PGY-3)

The third year of the residency (first year of neurology training) is dedicated primarily to learning the fundamentals of neurology, neuroscience and neuroanatomy. Most clinical experiences of this first year of neurology training are spent on adult neurology rotations—a combination of inpatient (4 blocks), outpatient (3 blocks) and adult electives (3 blocks). Residents rotate onto the child neurology service during the final 2 blocks of the first year.

Inpatient experiences include adult neurology ward and consult services. Electives include adult epilepsy, adult stroke and neuro-ICU. Outpatient options include neuro-oncology, neuro-ophthalmology, neuro- immunology, EMG/neuromuscular, headache and movement disorders.

Postgraduate Year 4 (PGY-4)

The majority of the fourth year of the residency (the second year of neurology training) is dedicated to the inpatient child neurology service. About one half of service time is spent as the senior supervising resident on the CHAM child neurology service, and the other half as the senior supervising resident of the Jacobi and Weiler sites.

In addition to the inpatient experience, there are a variety of outpatient experiences, including continuity clinic 1–2 times per week, epilepsy clinic, MDA/neuromuscular clinic, pediatric neuro-oncology clinic and, once a month, “private practice” with Dr. Moshe.

Postgraduate Year 5 (PGY-5)

During the fifth year of the residency (third year of neurology training), the child neurology resident consolidates their knowledge in both adult and pediatric neurology and functions as a soon-to-be expert. The resident has the opportunity to explore research or other electives and tailor their learning experiences to their professional goals. Research electives must be discussed with and approved by the Program Director or Division Chief and carry additional expectations for research productivity.
