CHAM_Rounding_2010_10_065_316x210When you or your child needs medical care, you don’t want to wait around: you need high-quality, convenient care, suited to your schedule. That’s why the Division of Academic General Pediatrics is linked up with other pediatric care centers in the area, to provide maximum coverage for our patients.

The Comprehensive Family Care Center (CFCC) and The Family Care Center (FCC) offer high quality, convenient, multidisciplinary care to residents of the Bronx and beyond.

Both sites have attained Level Three Patient-Centered Medical Home (PCMH) certification from the National Committee for Quality Assurance (NCQA)—that’s the highest ranking available. That means that as a patient, you or your child will receive patient-friendly, easy to access care of the highest caliber. Both the CFCC and the FCC are part of the ongoing Montefiore Medical Group learning collaborative to improve care in the areas of:

  • Access for teens (Healthcare for children and adolescents)
  • Immunizations
  • Asthma management
  • Coordination of care
  • Patient satisfaction
  • Experience of care

Preventing Illness Early Through Screening

A part of a large grant Bronx Ongoing Pediatric Screening, these sites offer routine screening for developmental and socio-emotional disorders in childhood: at 12,18, 24, and 36 months, followed by annual visits. For adolescents, sexually transmitted disease and mental health screening is available.

Both participate in the Healthy Steps program, designed to ensure that parents and their babies get started on the right track with regular check-ups, a dedicated parent hotline, educational literature, optional at home visits, parent discussion groups, referrals to children’s specialists, and on-site mental health treatment.

Enroll your little one in the Reach Out and Read program at either site to promote your child’s early literacy. Your little one will receive an age-appropriate book from six months to three years at each visit.

The Behavioral Health Integration Program (B-HIP) at both the CFCC and the FCC is designed for children ages five and up, and provides onsite counseling and mental health services for those who screen positive. Staff includes a child psychologist and child psychiatrist.

Comprehensive Family Care Center (CFCC)

Whether your child is sick or just needs a check-up, our Comprehensive Family Care Center (CFCC) is a great place to receive care. The CFCC offers inclusive options for the entire family—so that yours can receive flexible, convenient treatment and access specialists when needed. 

Family Care Center (FCC)

As a busy parent, keeping your child healthy can feel like a full-time job. Our Family Care Center (FCC) puts CHAM’s high-quality care providers all in one place—so your family members can get the care they need, when they need it. 

Care of Children with Complex Chronic Medical Needs (LINCS)

When your child has multiple medical conditions that require ventilators or other medical equipment, navigating physicians' offices can be stressful and time-consuming. That's why the Children's Hospital at Montefiore (CHAM) provides a staff and patient area just for families with special needs children.

Community Pediatrics

The Community Pediatric Programs (CPP) offered by the Children's Hospital at Montefiore (CHAM) fulfill a vital, philanthropic mission.

JE and ZB Butler Child Advocacy Center

Like the New York Yankees, CHAM's Butler Child Advocacy Center is known around the world as a winning team—in our case, for the detection, treatment, and prevention of child abuse and domestic violence. The only fully accredited child abuse program in the Bronx, we provide emergency care and support to young victims of abuse and neglect, crisis counseling to children and families, and extended support until everyone recovers.
