Comprehensive Discharge Planning Program
Seeing your infant through those first crucial days is indescribable. But birth is just the beginning-that's why CHAM offers a Comprehensive Discharge Planning Program for new parents. Our program allows parents to plan for the months and years to come, educate themselves on how to best care for their baby, and look out for signs of illness down the line.
Because we take a multidisciplinary approach, our planning process coordinates both discharge and post-hospital services. We offer:
- Parent education, including infant CPR training
- Rooming-in for parents who need to practice special care skills before bringing their newborn home
- Referrals to community agencies and follow-up services, including to children's rehabilitation programs at Blythedale Children's Hospital, Elizabeth Seton Pediatric Center and St. Mary's Hospital
- Referrals to primary care pediatricians and pediatric specialists
- Home monitoring and specialized equipment
- Home health nurse visitation (Montefiore Home Care, Visiting Nurse Service) and extended home nursing
- SYNAGIS shots for very premature babies who are at risk for RSV infection
- Breastfeeding and lactation support
- Referral for early intervention for infants demonstrating developmental delays
