The Division of Community & Population Health supports faculty and trainees across Montefiore Einstein through both community health initiatives and embedded research. The Division aims to:

address health related social needs and reduce health inequity
optimize the integration of community health workers in health care delivery
embed learning health system approaches in real-world practice

We support two teams, the Community Health Institute at Montefiore Einstein and the Community Health Systems Lab.

The Community Health Worker Institute at Montefiore Einstein aims to optimize community health worker integration, value and effectiveness to address health related social needs and to provide Bronx- based households career opportunities.

The Community Health Systems Lab (CHSL) is an interdisciplinary team of researchers, providers, and frontline staff that supports health systems strengthening and implementation research in the Bronx and West Africa

We use implementation science and community expertise to develop and deliver interventions to improve community health. Our team of faculty researchers, data analysts, and community experts are engaged with partners across Montefiore Einstein and with national and global collaborators. Across all of our projects and geographies, our shared mission is to deliver high quality health care and reduce health inequity.
